Health Requirements & Info


Healthy students are better learners. We are here to help support your child’s health and wellness. Please contact health services staff (see below) to let us know of any health concerns or medical conditions that your child may have. Remember, if your child is sick, please keep them home. See the "When To Stay Home" guidelines below.

Please call the school attendance line each day your child is absent due to illness.

Does your child have a chronic health condition we should know about? Please click the link below to access the appropriate forms. Once completed, return the forms to the health staff. We will work with you to develop a plan to keep your child healthy at school. Forms must be updated each year.

The school district recognizes that parents and guardians have a primary and fundamental role in promoting and protecting their children’s health and well-being. The school district will support parents’ efforts to provide a healthy diet and daily physical activity for students. We ask for your help in supporting these efforts to ensure your child's physical health and wellness. See our district Wellness Policy R 1.2.2.
It is the responsibility of the parent/ guardian to notify the appropriate school staff of any medical condition that may impact the health or safety of their child during the school day or in school activities.
